District requirements by grade level

New students:  Immunization records as well as grade specific requirements 

Preschool: birth certificate, immunization record, dental screening, and lead screening

Kindergarten:  birth certificate, immunization record, dental screening, vision screening,  and lead screening 

3rd Grade: vision screening

7th Grade: Tetanus, Diphtheria, and Pertussis (Tdap) and Meningococcal vaccination.  There is no provisional period to implement this requirement, meaning students will be sent home on the first day of school.  This is a state requirement. 

9th Grade - Dental screening completed by a registered dental hygienist or dentist.

12th Grade:  Meningococcal vaccination.  There is no provisional period to implement this requirement, meaning students will be sent home on the first day of school.  This is a state requirement.
