School Permit

Following the completion of Driver's Education, a student and parent/guardian need to complete an application for a school permit. This form must be completed prior to seeking a signature from Dr. Nelson. Paper copies of these forms are available in the entryway of the Administration Office at 304 W. Nishna Rd.  This building is open 8AM-3PM Monday through Friday.  The fields that need to be completed are highlighted in orange.  If the student lives less than one mile from the school, a statement describing the circumstances that create a need for the student to obtain a school permit is required when seeking approval.  Please bring a photocopy of the student's Instruction Permit or our office staff can make a copy.

Once the form has been signed by Dr. Nelson, the form will need to be taken to the DMV.  The Shenandoah site on Wednesday is currently closed.  The Clarinda site is open by appointment only.  You may call (712) 542-5322 to request an appointment at the Page County Courthouse.  The Sidney site is also open by appointment only.  You may call (712) 374-2122 to request an appointment at the Fremont County Courthouse.  Applications can also be submitted electronically by emailing the completed application to remote.issuance@iowadot.us.  It is requested that you include your phone number in the email, as you will receive a call to complete processing.  The electronic process can take up to 14 business days to complete.
